How to Join

If you want to join the practice please read the guide below for joining as a new patient

To Register:

  • Complete a GMS1 Registration Form
  • Complete a Pre Registration Form
  • You do not need to bring any ID or proof of address or immigration status
  • If you have a medical card please bring it with you as the information on the card will speed up the process 
  • You do not need to inform your existing practice that you are moving, although it may be polite to do so.

Please bring the completed form along to reception or email the completed forms to nnicb-nn.cvamp@nhs.net

Non-urgent advice: Notice

If you are on regular tablets or medication when you join the practice then our practice pharmacist will contact you on your preferred telephone number to discuss this with you.

You do NOT need to make an appointment just for this

Out of Area Patients

It is practice and Integrated Care Board (ICB) policy that patients living at an address which is outside the area agreed with our ICB from which we take patients need to register with another doctor.

Due to increased demand on our service we are reviewing our patient list and in order to be able to safely care for our local population we need to ask that such patients register with a practice which covers that area in which they now live.

This decision has been made by the partners of the practice after much discussion and, while we all regret that this action is necessary, we do need to let patients know that it is not negotiable and neither the doctors nor our staff are able to make exceptions to this partnership decision.

If you have been directed to this page, please register with a local practice within the next 30 days. After this time we regret that your name will be removed from our list.
