The Nottinghamshire Help Yourself site is a partnership between health, the voluntary sector and Nottinghamshire County Council to bring information and advice together in one central place – so people can find the information they need easily.

Connect offer free practical advice and information and can help with the following:

  • Signposting to local organisations who best meet the patients needs
  • Boosting confidence to get out and about in the community
  • Building friendships
  • Support to attend appointments
  • Completing forms for benefits, housing etc
  • Offer up to 3 months tailored support
  • Bereavement
