Medication to Delay your Period

Unfortunately our practice no longer provides prescriptions of hormones to delay periods for holidays. This is not part of the work of the NHS.

If this is a a service you would like it can be provided by some pharmacists after assessment at a cost of £15-20.

Completion of forms and certificates by a doctor.

When you are registered with a doctor under the NHS most of your routine care is provided free of charge. This also covers things like providing standard sick or fit notes and completion of some forms, such as for claiming benefits.

Many other types of medical forms and certificates are for other purposes such as insurance claims, or for employers. Completion of these is not part of a doctor’s work under the NHS and we therefore make a charge for doing them. Below we have tried to answer some of the questions that are commonly asked about forms and certificates issued for private purposes.

Why do I have to pay for a form or certificate to be completed?

Because the work involved is not covered under the NHS and is usually for private purposes. It is always worth checking with the person or organisation that has asked you to get the form completed to see whether they will pay the relevant fee.

Why do the fees seem so expensive?

Because you have been asked to obtain details from the doctor as a professional person, the fees are comparable with those charged by other professionals such as lawyers or accountants. They also take into account administrative time and overheads.

If it is only a short form why might it take so long to get completed?

Any form or certificate which you ask your doctor to complete is only one of many which they get each week. Doctors have to spend two or three hours a week doing this sort of work outside normal working hours. This is to make sure that it does not affect the care offered to patients requiring medical attention. However, if there is a deadline for the form to be completed then please let us know and we will try to meet this.

Shotgun and firearms certificate requests

Chilwell Valley and Meadows Practice does not provide medical reports for shotgun or firearms certificates. This work falls outside the NHS General Medical Services (GMS) contract and is therefore considered private work. Due to current workload pressures and our commitment to prioritising NHS patient care, we are unable to undertake this service.

Patients seeking a medical report for a firearms or shotgun certificate will need to arrange this through a private GP or an alternative provider. 

Other private services

Some private services require much longer time to complete which we do not have the capacity to accommodate, so if the service is not listed in the summary of fees below then this will be the reason.

We do not do housing application support letters.

Please note that payment for these services is to be made upfront. We are sorry but we do not accept card payments.

Screening Services

Mammograms are offered to all women between the ages of 50 and 70. All our female patients are invited every 3 years. So your first mammogram will not necessarily be immediately after you turn 50. If you think you have missed being screened due to moving area, or if you wish to continue being screened beyond the age of 70 please contact the breast screening unit on 0115 993 6649 to arrange an appointment.

NHS bowel cancer screening is only offered to people aged 60 or over, as this is when you’re more likely to get bowel cancer: if you’re 60 to 74, you’ll automatically be invited to do a home testing kit every 2 years. If you’re 75 or over, you can ask for a home testing kit every 2 years by calling the free bowel cancer screening helpline on 0800 707 60 60

Health and Well-being Coach

The Health and Wellbeing Service aims to help patients in the Nottingham West Primary Care Network take more of an active role in their own health care through structured Health Coaching consultations. There is a focus on changing detrimental lifestyle behaviours with the intent of reducing health risks, improving self- management and symptoms of chronic conditions.

The Health and Well-being  Coach can guide and support patients to set realistic and achievable health related goals to improve quality of life and overall health and wellbeing. The criteria for the service includes all patients living with Diabetes or pre-diabetes and all patients living with a respiratory condition. The Health Coach offers 1:1 support as well as group exercise sessions and support sessions. Sessions can be conducted virtually, face to face or over the phone according to the patients preference.

Flu Clinics

There is an annual clinic held on a Saturday morning in October for this. Otherwise an appointment can be made with the practice nurse. Current recommendations are that anyone over 65 or suffering from heart, kidney or lung conditions should have an annual flu immunisation. A pneumonia immunisation is also available for these individuals but does not usually need repeating.

NHS Health Checks

We are offering health checks for people aged 40-70 every 5 years which includes blood tests for cholesterol and blood sugar. We are currently calling in people for this service and we will be in touch.

Health Promotion and long term condition management

We are committed to improving the health of our patients. A lot of services in Nottingham will take referrals directly from patients without needing to see a doctor or nurse.

The practice will get in touch with you , usually by text when your long term condition monitoring is due. In order to prioritise these we may send you a questionnaire or arrange for blood tests to ensure we pick up changes in your condition.

Liberty Leisure offer an exercise referral scheme which is a discounted gym membership with extra support to help people with low level mental or physical health conditions to improve their lifestyle through regular physical activity. They offer hand-holding support to help people along this journey with the aim to enable people to do this as part of their lifestyle independently. More details here.

Joint injections

At our practice we offer joint injections which are available at our clinics run by multiple doctors at both Valley and Chilwell Meadows surgeries. Travel Services / Vaccinations

You need to be a registered patient at the practice to receive travel advice from us.

Non-urgent advice: Notice

Non NHS vaccinations such as rabies, Japanese encephalitis etc are not offered by the surgery. These can be accessed locally at private travel clinics.

NHS vaccinations that we do offer are: Diphtheria, tetanus, polio, measles, mumps, rubella, hepatitis A and typhoid and cholera if indicated.

We are not a yellow fever centre and do not offer yellow fever vaccination. Further Information regarding vaccinations is available at the NHS travel vaccinations website.

If you are travelling abroad please ensure you contact us with at least 8 weeks notice before your planned departure date. Alternatively you are able to go to a private travel clinic locally.

All completed electronic questionnaires should be received by the surgery a minimum of 8 weeks ahead of your departure date.

Once we have received your response we will then contact you in due course to discuss your travel and arrange an appointment if you need any vaccinations.

Please ensure you have given us a correct and up to date daytime contact number and a preferred time to be contacted.

If we ask you to come in for an appointment you will need to bring a printout of the travel advice specific to any countries that you are visiting from the Travel Health Pro website.

We no longer give out printed information. A face to face appointment will only be offered if vaccinations are advised.

The Foreign and Commonwealth office (FCO) is available which has the latest advice by country for safety, security, entry requirements and travel warnings.

You can get healthcare cover abroad with a GHIC or EHIC card.

A face to face appointment will only be offered if vaccinations are advised.

If you are travelling in less than 8 weeks you can still check out the information from the above details but you will need to get specific travel advice along with any required vaccinations privately.

If this is the case the practice is happy to provide you with a list of any vaccinations which you have previously received.
