Non-urgent advice: Having Problems?
Access going forward from 1st April 2023
The NHS wants to give people better ways to see their personal health information online. We know that people want to be able to access their health records.
It can help you see test results faster. It also lets you read and review notes from your appointments in your own time.
We’re now letting you see all the information within your health record automatically. If you are over 16 and have an online account, such as through the NHS App, NHS website, or another online primary care service, you will now be able to see all future notes and health records from your doctor (GP).
Some people can already access this feature, this won’t change for you.
This means that you will be able to see notes from your appointments, as well as test results and any letters that are saved on your records. This only applies to records from your doctor (GP), not from hospitals or other specialists.
You will only be able to see information from 1st April 2023. For most people, access will be automatic, and you won’t need to do anything.
Your doctor (GP) may talk to you to discuss test results before you are able to see some of your information on the app. Your doctor (GP) may also talk to you before your full records access is given to make sure that having access is of benefit to you.
There might be some sensitive information on your record, so you should talk to your doctor if you have any concerns.
If you are a carer and would like to see information about someone you care for, please refer to the section below about accessing online services on behalf of someone else (Proxy access).
The NHS App, website and other online services are all very secure, so no one is able to access your information except you. You’ll need to make sure you protect your login details.
Don’t share your password with anyone as they will then have access to your personal information.
If you do not want to see your health record, or if you would like more information about these changes, please speak to your GP or reception staff.
If you have access to your online records and wish to request an amendment due to a factual inaccuracy please complete this form. Please read the guidance about this here.

Access for young people aged 13 to 15 years
A young person aged between 13 and 15 inclusive may apply for online access on their own behalf (appointments and prescriptions only) and such requests will be considered by a GP on a case by case basis. Click on this link for the form.
Accessing online services on behalf of someone else (Proxy access)
If you have a child under the age of 11 then you can apply to have online access to services for them. You will be asked to provide evidence that you have legal responsibility for them e.g. a birth certificate
Young people may occasionally wish certain medical information to be kept confidential, even from their parents. For this reason the NHS automatically turns off access to all online services by parents of all children at the age of 11. A young person aged over 13 may apply for online access on their own behalf (appointments and prescriptions only).
We will consider requests for online access by a third party in other circumstances, such as by carers of patients with learning disability or dementia. However, we would still expect the patient to agree to this where they are capable of some understanding. This will be for prescriptions and online appointments only.

Subject Access Requests
How do I apply for access to my medical records (“Subject Access Requests”)?
In many cases if you just want to see your medical record online the simplest way is to register for the NHS App (see above paragraphs).
However if you would like more complete access (a report) this is called a Subject Access Request (SAR).
To make a Subject Access Request please download a form here and return it to reception or email it to [email protected].
Our practice has decided to outsource our medical reporting to iGPR, who will process your medical report using their system.
Online queries
Our online queries access has been available since 2023. This allows you to contact us at any time directly about anything which is non urgent and requires our help. Please follow this link or the image below. Please bear in mind that it may take up to a week to answer the query if it is for a particular doctor.
