Diabetes Support Group
We are a group of Health Professionals who work with people living with Diabetes. We are keen to explore any issues you face and offer advice and support.
Do you sometimes feel overwhelmed or isolated? Or would you welcome an opportunity to talk to others over a cup of tea or coffee? Well, this might be the place for you!
Please click on this link to find out why we no longer prescribe these.
Want to book appointments, order repeat medications, see test results or view your medical record online, but you’re not sure how?
The NHS app is the answer and you can sign up for it yourself, or if you would like some help there are drop-in clinics with Digital Inclusion Officers. Please click on this link to find out more details.
Hello. Welcome to the Chilwell Valley and Meadows Practice. We are a two site practice run by Chilwell Valley and Meadows Partnership, a partnership of 12 GPs and an active member of Nottingham West Primary Care Network. Note that some information on this website is site-specific.
Please can you order all your medication ONLINE via the NHS App, or SystmOnline as this is the safest way to ensure your medications are correct. All prescriptions are sent electronically to a local pharmacy of your choice. Please choose one if you have not done so.
If you cannot order on line please drop your prescription slip into the box at the side of the surgery or if TOTALLY housebound and no one can do this for you phone the surgery between 10am and 12noon.

We are now offering a mixture of face to face and telephone appointments to try to suit all your needs and requests. We are constantly working to improve our appointments and are offering 19% more appointments than we did before the pandemic.
Telephone appointments fit around our face to face appointments so may not be at an exact time and you will need to keep your mobile by you.
Appointments are released every day, please bear in mind that Mondays are always the busiest day of the week for urgent appointments and mornings are always more busy on the telephone than the afternoons.
Routine appointments for doctors are available online and we can send you an online booking link for many of our nurse clinics.
Our reception staff are trained to support you to get the help that you need in the best way possible so they will continue to ask for a brief outline of the nature of your issue. They have the same standards of confidentiality as doctors. If you feel unable to give them any information they will respect this.
We want everyone to feel safe coming to our practice so we encourage mask wearing in the waiting room for those who are clinically vulnerable and for anybody coming to the surgery with an infectious respiratory condition for example tonsillitis, new cough or cold.
Clinical staff may continue to wear masks in the surgery as an individual choice but if our wearing a mask is a problem for communication reasons please let us know.
Long term condition reviews
We will contact you by text or telephone to arrange your annual review with the appropriate nurse.
You can contact us anytime if you have concerns or queries about your medications. An appropriately trained doctor, nurse or clinical pharmacist will review your medications at least yearly and discuss them with you if appropriate to do so.
Blood pressure monitoring
Local pharmacists are now paid to carry out blood pressure checks on anybody aged 40 and over if you do not have an existing diagnosis of raised blood pressure. Please can you give us a copy of these results for your records.
From 1st April 2023: Access to your medical records.
You can now view your blood test results, hospital letters and GP consultations on the NHS app from April 1st. You can order medications and send us a message on the NHS app as well.
It is very important that if you have any security concerns about your confidential information being available on an app on your phone or laptop and do not want this information accessible please let us know in writing preferably or at reception.
If you are struggling with the NHS app there are people who can give you personal help with it. These people are called Digital Inclusion Officers.
You can speak to reception who can give you a mobile number to ring.
Drop in help sessions also take place at the following places and times:
Tuesdays 09:00-12:00 at Stapleford Care Centre, Church Street Stapleford (NG9 8GA)
Wednesdays 09:30-11:00 at Hope House, Boundary Road Beeston (NG9 2RF)
Thursdays 10:00-12:00 at Middle Street Resource Centre Beeston (NG9 2AR)
Thursdays 13:00-15:00 at The Beeston Social under the ARC Cinema (NG9 2WJ)
The surgery will remain open during any NHS strikes, please keep your appointments with us unless you are informed anything different. This also applies to hospital appointments.
We are offering a spring covid booster clinic on Saturday 27th April. It will be in the morning at Valley Surgery and afternoon at Chilwell Meadows Surgery. It will be for people aged 75 and over and those aged 6 months and over who are immunocompromised. We will send an invitation text closer to the time if you are eligible.
